What is ASTM E1394?

ASTM E1394 (formally 'Standard Specification for Transferring Information Between Clinical Instruments and Computer Systems') is an ANSI/ASTM standard defining how laboratory analyzers communicate with host systems over a serial RS-232 connection. It defines the physical layer (RS-232), the link layer (ENQ/ACK/EOT handshake), and the application layer (H|P|O|R|L|C|Q record types).

Why it matters for your lab

The vast majority of clinical lab analyzers — haematology counters, chemistry analyzers, immunoassay systems — use ASTM E1394 as their native communication protocol. Without middleware that correctly implements the full ASTM spec, labs rely on manual transcription of results — a process proven to introduce errors at a rate of 1 in 300 keystrokes. LISBridge eliminates that entirely.

How LISBridge handles ASTM E1394

LISBridge implements the complete ASTM layer-2 handshake: it responds to ENQ with ACK, handles STX/ETX frame boundaries, processes each record type (H, P, O, R, L, C, Q), sends ACK after EOT, and handles retransmission correctly. Instrument-specific quirks — COBAS 15-second timeout windows, Mindray multi-record reassembly, Sysmex delimiter variations — are handled by instrument-specific parsers loaded automatically.

What LISBridge Does

🔌

Full ENQ/ACK/EOT handshake

LISBridge responds to every ENQ with the correct ACK, handles frame boundaries, and sends the required EOT acknowledgement — per the ASTM spec.

📋

All record types

H (header), P (patient), O (order), R (result), L (terminator), C (comment), Q (query) — all parsed and mapped to structured output.

⚡

Real-time processing

Every byte arrives off the RS-232 wire and is parsed in real time. No buffering delays. Results reach your HIS within seconds of the instrument finishing.

🔄

Instrument-specific parsers

26+ instrument-specific parsers handle proprietary ASTM extensions, delimiter variations, and timing quirks unique to each manufacturer.

🛡️

Validation on every message

Incomplete frames, checksum errors, and duplicate transmissions are caught before any data moves forward.

♻️

Auto-updating via WASM

Parser improvements and new instrument support are delivered via CDN every 15 minutes — no reinstall, no downtime.

Frequently Asked Questions

What is the difference between ASTM E1394 and HL7?

ASTM E1394 is a serial RS-232 protocol designed specifically for lab instruments communicating with a local computer. HL7 is a TCP/IP-based messaging standard designed for communication between hospital systems. Most lab analyzers use ASTM internally; HL7 is typically used to send data from the middleware to the HIS. LISBridge handles both.

Which instruments use ASTM E1394?

The vast majority of clinical analyzers — including Sysmex, Mindray, COBAS, Horiba, Dimension, ADVIA, Laura, and many others — use ASTM E1394 as their native protocol over RS-232. Some also support TCP/IP variants.

Do I need to configure the ASTM settings in LISBridge?

No. LISBridge reads the first bytes of the ASTM stream and auto-detects the instrument model, then loads the correct parser automatically. You don't need to select an instrument or configure delimiters manually.

Does LISBridge support ASTM E1381 as well?

ASTM E1381 defines the lower-level RS-232 connection and framing. LISBridge implements both layers — E1381 for physical connection and E1394 for application-layer records — as a complete stack.
